Office Hours

Our office hours are 8:00 a.m. to 5:00 p.m., Monday through Friday. Be assured that we provide emergency coverage 24 hours per day, 7 days a week.

Initial Visit

On your initial visit, please bring the following items:

• Identification
• Insurance card(s)
• Written referral form, if required
• List of medications you are taking
• Pertinent test results

Also, be prepared to share your medical history. This will include any medical conditions you have or may have had in the past, surgical history, any medications (prescription or over-the-counter) you are currently taking (include name, dosage and frequency), and any allergies you have to medications or foods.
